Six Sigma was pioneered by Motorola in the mid-1980s and has spread to many other organizations. The term literally translates into 3.4 defects per million or 99.9997%.

According to an OfficeTeam survey of 150 senior executives at the nation's 1,000 largest companies, 71 percent listed e-mail as their most common form of dialogue at work, compared with 27 percent five years ago.
